All Students must have a current physical on record in the health office inorder to tryout or participate in sports. If the current physical is due to expire during the season of the sport, a new physical must be obtained and recorded in the health office. Sport physicals are valid for one calendar year and must be submitted to the health office, not to the coach or the athletic office. Physicals are given in school by the district's Family Nurse Practitioner at no cost. Please schedule an appointment with the health office. All students not participating in a school sport, who are entering seventh grade must have a physical examination form completed within 12 months prior to the beginning of seventh grade, but must be dated after the beginning of the sixth grade school year in which the examination is required and provided to the health office within 30 days after the student has started school. Otherwise, our school physician/nurse practitioner will conduct the physical.
Modified Sports Physicals
All students who would like to play a sport will need a current physical on file in the health office.

Students who require to be on crutches during school hours should enter the building by cafeteria B and go directly to the health office with instructions from their doctor. Students must have a note from their doctor in order to use crutches in school. The student will be issued a pass to be excused from gym class and sent to the guidance office for a PE study hall until medically cleared by their doctor. The student will also be issued a pass to use the elevator and to leave class 5 minutes early with an escort; preferably someone going to the same class. Students on crutches are to refrain from using the stairs and must stay indoors for recess.
If your child is taking a prescription medication or an over the counter medication, an "Authorization for Administration of Medicine," form must be completed by the parent/guardian and the physician prescribing the medication. All forms must be completed in their entirety and returned to the health office. No medication will be administered without written consent. If you need medication in school, it is to be left in the health office. All medication brought into the health office must be in the original container and delivered to the health office by the parent/guardian.
Any student unable to participate in gym class due to an illness or an injury must have a written note from a doctor. In order for the student to return to gym class, the student must a clearance letter from the doctor. A parent/guardian may excuse a child for ONE day only in writing. All notes go to the health office first.
Any student on a PE Medical is unable to participate in sports until medically cleared. Students diagnosed with a concussion must complete a five day return to play protocol once cleared by a physician before returning to gym/sports.
All students will be assigned to a P.E. Study Hall until he/she may return to gym class.
